How to Request Medical Records from Integrated Services of Kalamazoo

Per ISK policy all requests for records must be in writing and submitted to our Medical Records Department. ISK Medical Records is located at our Administrative Offices at 610 S. Burdick Street, Kalamazoo, MI 49007

If you would like to request your own records, please complete the Patient Records Release Request, and submit to our offices for processing. If you would like to have your records sent to another facility, please complete the ISK MDHHS Release Consent to Share for ISK to either request previous treatment records and/or coordinate your care to your new treating provider.
