Every employee throughout Integrated Services of Kalamazoo and the contracted provider network is responsible for reporting any act of fraud or abuse. In fact, if you know of a wrongful act and you fail to report it, you too could be held responsible. Employees are protected as “whistleblowers” by the False Claims Act against retaliation for reporting a concern in good faith about a suspected violation. All reports will remain confidential and can be filed in person, in writing or verbally.
To report suspected compliance violations, you can use ISK’s Compliance Violation Reporting Form (Procedure Exhibit 10.02_01B). To access it, click here, then click the “+” sign next to “Provider PDFs” and select “Exhibits.” Then find and select Exhibit 10.02_01B.

For Community Members: Individuals Receiving Services, Family, Etc.
Members of the community may report compliance allegations and concerns even if they’re not an ISK employee or contracted provider.
If you suspect any fraud, waste, or abuse of public funding, you can file a report using ISK’s Compliance Stakeholder Complaint Notification Form (Procedure Exhibit 10.02_01D).
