ISK’s Healthy Transitions Program works with youth and young adults ages 16-25, with either a Serious Emotional Disturbance (SED) or a Serious Mental Illness (SMI).

Transitioning into adulthood represents a particularly challenging period for youth and young adults. The Transition Program prepares and supports youth and young adults with:

  • Moving into employment
  • Educational opportunities
  • Living situations
  • Personal effectiveness/well-being
  • Community-life functioning

Transition Peer Support Specialists work to connect with the youth through shared lived experiences, empower youth to be independent, and support youth voice and choice in their own treatment. They assist youth with overcoming transition barriers and challenges and translating them into skills.

How do I receive services?

The first step to receiving services is to contact our Access Center at 269-373-6000 . The staff at the Access Center will do an initial screening to determine what services you qualify for and connect you to those services.

Cost for services

Fees are based upon our Sliding Fee Discount Program or your health insurance coverage. While no one is denied services based on their financial situation, there may be a waiting list in place to receive non-Medicaid covered services.
