SAME-DAY ACCESS (non-urgent):
ADULTS: Monday-Friday 8 a.m. to 8 p.m.
YOUTH: Monday-Friday 8 a.m. to 5 p.m
SATURDAY (Youth & Adults): 9 a.m. to 2 p.m.
WHAT TO BRING
If possible, please bring with you:
• IDENTIFICATION
• INSURANCE CARD
• GUARDIAN (if applicable) and PROOF of guardianship
• If you plan to seek discounted services, please also bring a W-2, CURRENT PAY STUB, BANK STATEMENT, or other proof of income.*
